This is an order form for a Gateway Sub order to arrive at the tournament by 12pm on Sunday, March 1st.
 
All orders must be placed by Friday, February 27th at 12pm.
 
Whole Subs: $14 plus fees
Half Subs: $9 plus fees
 
Meat Subs Include: Provolone Cheese, Lettuce, Tomato, Onion
 
Veggie (Vegan) Subs Include: Cucumber, Spinach, Lettuce, Tomato, Onion, Black Olives, Avocado Spread 
 
Mayo & mustard packets will be provided in each bag.
